Time To Register! Registration is easy. However it is only step 1 of 3 towards becoming a certified volunteer! Please complete the form below and submit it. After this step you'll need to attend a training session. All fields must be filled in! ↓ FILL OUT ↓ ↓ READ FIRST ↓ First Name: Names must match that which you are registered under at the university to ensure you receive Cincinnatus hours if you request them. Only letters and hyphens ("-" dashes) are accepted. Middle Initial :   Last Name: (Same rules as for first name.) eMail Address: Only submit the email address which you check most often! Phone Number: Only submit a phone number which you can consistently be reached at throughout the day! (Usually a cell phone). DO NOT include dashes. DO include the area code. (i.e. 4195529639) Full Local Address: This can be a dorm name & room number or a full address of where you are currently living while attending UC.       Do you have a valid US drivers license?   Yes: No: You must have a valid US license to drive a van. Have you had any moving violations on your driving record in the past 6 months**?   Yes: No: Driving history background check will be performed. Would you like us to automatically submit your volunteer hours for Cincinnatus?   Yes: No:   If you need hours for Cincinnatus you must give us your student ID # below. Leave student ID # empty if you do not need Cincinnatus. Student ID: Include the letter "M" at the beginning. Example: M02286698 ** If you have any recent history of moving violations as described above you will not be allowed to drive any university vehicles. ** Please specify a username and password to access your profile... Username: Pick a simple but unique username. Valid characters include letters, numbers, periods (.), and underscores (_).       Password: Specify a password you can remember.
